So if I am to get Vizard ICly what do i apply as? shinigami or vizard?? Also where would i submit my mask form?

Then you would undergo hollowification at some point.

And should you succeed hollowification, I think you would request an improvement and I guess you would use the Vizard template and carry over your information along with your newly obtained hollow powers.

It would probably be best to wait for Empty/Mazra/Leon/Ami/Bear to respond if you want a clear and precise answer.
